Wednesday 25 November 2015 was a very dark day in Western Australia with three tragic, preventable fatalities. The victims were two Irish ceiling fixers both in their 20’s and a 32 year old scaffolder.
1st incident
A truck load of precast panels loaded on A frames was parked on the road in front of the eleven storey apartment block being built by Jaxon construction. The rigging crew identified that the panels were loaded out of sequence. Three panels at the front (kerbside) of the truck overbalanced and fell in toward the building. Two of the panels came to rest against the upright column of the gantry the third panel fell between the columns crushing the two young ceiling fixers who were sitting on a concrete plinth having a smoke with approximately eight other workers.
